Overview videos to describe revisions to special education forms

Forms to be used throughout the special education process have been developed by the Pennsylvania Department of Education to ensure that students with disabilities receive a free, appropriate, public education. Forms are available for the Infant-Toddler, Preschool, and School-age programs, and include both annotated and non-annotated versions.


 School Age Forms

These special education forms for school age programs are developed by the Pennsylvania Department of Education to ensure that students with disabilities receive a free, appropriate, public education. Non-annotated forms have been translated into multiple languages.

Preschool Forms 

These special education forms for preschool programs are developed by the Pennsylvania Department of Education to ensure that children with disabilities receive a free, appropriate, public education. Non-annotated forms have been translated into multiple languages.

Infant/Toddler Forms 

These special education forms for infant/toddler programs are developed by the Pennsylvania Department of Education to ensure that children with disabilities receive a free, appropriate, public education. Non-annotated forms have been translated into multiple languages.


Gifted Education Forms

Chapter 16 of the PA Code specifies how the Commonwealth will meet its obligations to suspected and identified gifted students who require gifted education to reach their potential. It is the intent of the Board that gifted students be provided with quality gifted education services and programs. Gifted Education forms, as well as additional Information regarding responsibilities placed upon school districts with regard to the provision of gifted education can be found at  PDE’s Gifted Education Page.

 

Office for Dispute Resolution Forms

The Office for Dispute Resolution offers many services to interested parties. Presently, requests can be made for Due Process, Mediation, IEP Facilitation, and Resolution Meeting Facilitation.  

Request forms are available in both Spanish and English. For online submission, a form must be completed in its entirety with all required fields completed. Consultline specialists are available to assist individuals with the completion of the Mediation and IEP Facilitation forms.
