This session will provide participants with an understanding of the Pennsylvania’s post school outcome survey, My Plan 4 Success (MP4S) and the requirements for implementation. The purpose of MP4S is to determine, for the state, the extent to which students are achieving transition outcomes as stated in their Individualized Education Programs (IEPs). This is a required training for identified LEAs.

Target Audience

LEA/APS is required to attend this meeting.

We also encourage the participation of:
Special Education Directors; Special Education Supervisors; Intermediate Unit Transition Consultants; District Transition Coordinators; Middle School Special Education Lead Teachers; High School Special Education Lead Teachers; Building Principals; Guidance Counselors; School Psychologists; CTE Teachers; Regular Education Teachers; Other educational staff involved in secondary education.
